First Visit Information

Complete the Intake Form BEFORE your appointment
Once you schedule your appointment, we will EMAIL YOU A LINK to complete your "New Patient Intake Form" Online.
It consists of 4 pages. It will ask for your personal information, medical history, the reason for your visit, organ system function questions, and informed consent to care.
* It can take 15 to 20 minutes to complete your Intake Form.

* The Online Intake Form will ask you for your Credit Card information. You will not be charged anything until the time of your appointment.
​Another Option: You can arrive 15-20 minutes early to your appointment to complete the Paper Version of the Intake Form in the waiting room.

What to expect on Day 1?
The practitioner will ask you about the reason for your visit, review your Intake Form, and perform a diagnostic assessment. Then the practitioner will explain what they can do for you and the treatment process. After that, you will be informed of some of our policies. Finally, you will begin your first treatment.

What to expect after Day 1?
​
Based on how you respond during your response test treatments and if you require more than a few visits to achieve your desired health goal, the practitioner may recommend a personalized treatment plan consisting of the practitioner's best recommendations to help you achieve the best results possible in the shortest amount of time.

Plan your day accordingly
For your first visit, expect to be in the clinic for up to 90 minutes.
Follow-up visits can take anywhere from 30 to 50 minutes depending on the services scheduled and what you need.
For the first visit please try to arrive about 15 to 20 minutes early to account for traffic, find the building, find parking, settle into the space, and fill out the Intake Form if you have not done so online.

How to prepare​​

What to wear? We try to perform all our treatments fully clothed.

Please wear something loose enough that we can roll up the sleeves and pant legs.

Also, please wear loose clothing that allows us to access the body part that needs to be treated.

If treating the neck/shoulder/upper back, preferably a tank top, or sports bra.

If treating the hips/legs, preferably boxers, shorts, or undergarments that make you feel comfortable.

What to eat? Make sure you have eaten a light meal or snack before arriving for your treatments.
​
What to avoid? Avoid caffeine, energy drinks, and smoking within a few hours of your treatment.

Caffeine, energy drinks, and nicotine can affect your pulse and heart rate, which are two important diagnostic tools we use in Chinese Medicine.

Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapist's day that could have been filled by another patient who needed it. As such, we require 24 hours notice for cancellations or changes to your appointment. If you cancel or miss your appointment, you will be charged the full cost of your scheduled appointment.

Late Arrival Policy
Should you arrive more than 10 minutes late to your appointment, a rescheduling/cancellation fee may apply. We'll do our best to accommodate late arrivals based on schedule availability and individual circumstances. Please inform us via text if you're running late, so we can adjust accordingly.​
