Helpful First Visit Information

Complete the intake form BEFORE your appointment
Once you schedule your appointment, we will EMAIL YOU A LINK to complete your "New Patient Intake Form" Online.
It consists of 4 pages. It will ask for your personal information, medical history, questions related to the reason for your visit, and informed consent to care.
* It can take 15 to 20 minutes to complete your Intake Form.

* The Online Intake Form will ask you for your Credit Card information. You will not be charged anything until the time of your appointment.
​Another Option: You can arrive 15-20 minutes early to your appointment to complete the Paper Version of the Intake Form in the waiting room.

Outline of what to expect on Day 1?
The Acupuncture Physician will first ask you questions about the reason for your visit, set goals, and do some diagnostic detective work to see what is going on. Then the practitioner will perform the treatment. After treatment, you will sit and chat with the Acupuncture Physician about the next steps of care.

Plan your day accordingly
For your first visit, expect to be in the clinic for up to 90 minutes.
Follow-up visits can take anywhere from 30 to 45 minutes depending on the services scheduled and what you need.
For the first visit please try to arrive about 15 to 20 minutes early to account for traffic, find the building, find parking, settle into the space, and fill out the Intake Form if you have not done so online.

How to Prepare for Your Appointments​
​​
Eat Light: Avoid heavy meals within 1 hour of treatment.
Stay Hydrated: Drink plenty of water before and after your session. Ideally, it should be room temperature or warm water.
Avoid Smoking or Caffeinated Drinks: This can make acupuncture less comfortable.
Wear Comfortable Clothes: Loose clothing makes accessing acupuncture points easier.
Please Silence Your Cell Phone & Relax: Arrive with an open mind and leave distractions behind.

We are on the 2nd floor. When you arrive on the 2nd floor you will enter the Flex-Works Waiting Room. Just have a seat and wait for the Acupuncture Physician to welcome you in or Text the office when you have arrived at (786) 309-1456
Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapist's day that could have been filled by another patient who needed it. As such, we require 24 hours notice for cancellations or changes to your appointment. If you cancel or miss your appointment, you will be charged the full cost of your scheduled appointment.

Late Arrival Policy

Should you arrive more than 10 minutes late to your appointment, a rescheduling/cancellation fee may apply. We'll do our best to accommodate late arrivals based on schedule availability and individual circumstances. Please inform us via text if you're running late, so we can adjust accordingly.​​
